Question: In order to produce a disruption report we will need a listing of all the current dentist being used by their tax ID number. If a dentist listing can be obtained we can produce a disruption report.

QUESTION 1. Is the current plan self-funded or fully insured? Are you looking for a self-funded or fully insured quote or both
RESPONSE: Lake County is currently fully-insured for Dental, and would be looking for the same.
QUESTION 2. Can you can provide the following information:
2 years monthly enrollment – 2 years of monthly paid premiums
Can we obtain monthly premium, so we can use against the monthly claims report?
Can we obtain a full dental booklet/certificate?
What are the current EE contribution levels?
