Contract No. 18168 - 18168 LCHD DENTAL LABORATORY SERVICES

It is the intent of the Lake County Health Department and Community Health Center (hereinafter “the Health Department”) to award Agreements for Dental Laboratory Services for the Health Department for Lake County. The Health Department reserves the right to award Agreements to multiple Contractors as a result of this RFP and to add or delete additional dental clinic site locations.
